HIV Program Referral
DOH-Brevard provides this service for HIV individuals new to the area and to those newly diagnosed or who have been out of care and need information about HIV services in Brevard County. Service referrals include medical, case management, housing, medication, insurance, social services, and others. Please call 321-690-6485 for more for information.
HIV Prevention and Education
HIV/AIDS education and prevention brochures, posters, and informative materials available. Condoms available in bulk for organizations and events. Can set up HIV/STD education presentations: general or specialized group and one-on-one individual meetings with positive and affected individuals. Available for events. Counseling and Testing
The Florida Department of Health in Brevard County provides counseling and testing on an Anonymous or Confidential basis.
Anonymous - no names are used so patients are provided anonymity. The patient is instructed to return to the clinic in two weeks for results and post-testing counseling.
Anonymous and Confidential Rapid HIV Testing with Confirmatory Test follow-up is also provided through the Community Health office. To make an appointment call 321-690-6486.
Confidential - counseling and testing is provided in our STD (sexually transmitted disease), Family planning, and Maternity clinics. To schedule an appointment for the Melbourne Clinic, call 321-726-2909. To schedule an appointment for the Viera or Titusville Clinic, call 321-637-7300.
HIV and AIDS Surveillance
The law requires that medical providers and laboratories report HIV+ and AIDS cases to the Florida Department of Health in Brevard County. Upon receiving the reports, each case is investigated and reported to the State Health Office in Tallahassee.
Linkage to Care
Linkage to Care is a service that identifies and monitors individuals as they transition into HIV medical care. Links newly HIV diagnosed or out of care individuals to early intervention services if needed.
Early Intervention Services
Early Intervention Services (EIS) increases a HIV positive individual’s awareness of their HIV status and facilitates access to the HIV care system and crisis needs interventions if needed. EIS focuses on health literacy and HIV medical education needs while linking and following the individual through the initial stages of HIV medical care, medication, and treatment adherence.
If you are a HIV positive individual out of care, you can contact our EIS coordinator for help accessing HIV care in our community or finding information about programs to address interventions that have kept you from getting HIV care.

AIDS Drug Assistance Program (ADAP)
ADAP provides antiretroviral medications to uninsured, underinsured and some insured individuals living with HIV. Services are provided through medication dispensed directly to eligible clients or by purchase of health insurance and medication copay assistance that includes coverage for HIV-related medications.
Through the online site https://rwces.floridahealth.gov clients can complete and submit the fillable online eligibility forms and/or documentation for ADAP Initial Enrollment and ADAP Recertification. Clients do not need login credentials to gain access to this site. The following forms are available for on-line submittal:

When completing a form(s) and/or submitting documentation on-line for ADAP Recertification please choose your ADAP coordinator for retrieval and review. For all ADAP Initial Enrollments, please choose Chanel.
ADAP Marketplace candidates are currently enrolled ADAP clients with incomes between 50%-400% of the Federal Poverty Level. Please contact your ADAP Coordinator for more information regarding eligibility.

This state funded program assists HIV agencies to provide services to Brevard County clients.
